    We are receiving denials on both MRA & MRI being billed together. They are being done during the same visit/DOS. We are billing 70546 70551-59 per NCCI edits 70551 is a column 2 code. IS this a common denial that we are just going to need to appeal?

    That was my conclusion as well, but what if ER admitted pt, we consulted, then pt was discharged would we will the admit/discharge code or would to discharging physician?

    I have a physician wanting us to bill 99235. 1. Do you have to be the admitting or discharging physician to bill this code? My example I do not believe we can bill 99235 since different calendar days. Pt was admitted by an internal medicine dr on 10/2/23. We consulted (neurology) on 10/3/23...

    We are trying to bill an ONB and trigger point injections. Is 64405 & 20552 billable together? I am reading the CCI edits I should bill 64405-59 20552. What is everyone seeing and billing for this?

    Wiki Would assessment qualify as the reason for encounter

    Encounter for botox for chronic migraine. Physician documents ot here for botox and then details the injection but does not mention chronic migraine or other dx why they are here until the end of the report under assessment would this hold up in an audit?

    We are billing for botox injections. The physician documents the pt is here for botox. Then details the procedure being done, then towards the bottom he documents assessment: chronic migraine. Would the assessment count as the reason for the visit?

    Wiki Capsule not reaching colon prior to completion of study

    We had an instance were a patient had a capsule endoscopy done, the report states normal small bowel was visualized, but the capsule did not reach the colon prior to completion of study. Billing wise- would this be reported as an incomplete? 91110-52 Thank you!
