    working for health center FQHC, if billing for a 99213 for professional and facility and also did xray 74046 would we need a modifier 25 on the e/m. we would bill S99213, T99213 and S74046- 26, T74046 - TC , but i wasnt sure if i needed a modifier 25 on the T99213? any help would be appreciated.

    For the prolonged code 99417 what is the time range for this for est. patient ..55-69min or is it 70 min. Does it have to be the full 15 min.past. Cpt book is 55-69min. but cms is 70 min. so im not sure which to go by. thanks for your help

    for telemedicine coding, if commercial insurance, can you use mdm or time . If they use time say 18 minutes. what cpt would you use.
