    Is this a code we can only use once on a patient? The patient was seen 6-9 months ago and we used 99455. We didn't do the hernia repair because of COVID. The patient came in this month to be re-evaluated for the hernia that happened at work. Can we use 99455 again?

    We have a patient that has bilateral breast implant. She is needing a left modified radical mastectomy for breast cancer and a prophylactic simple mastectomy on the right. Can I bill removal of the breast implants with code 19328 or is it considered part of the mastectomies?
