Wiki Diagnosing visits

If a diagnosis is in the "chief complaint" example: "Chief Complaints: finger pain and RA" BUT is not listed elsewhere in the note, can the RA be diagnosed? Most of my providers will make statements such as "patient is here for fu for rheumatoid arthritis" and continue with note and findings...

Hello! I have a provider who performed multiple procedures during 1 visit. Bilateral hand DIP and bilateral hand MCP. Any suggestions on how to bill this? (this is a UPMC insurance) I'm fearful we will be denied for duplication and not get paid. Thank you in advance!
