If the provider injected a mixture of .05cc of 5-FU (J9190) and .1cc of Kenalog (J3301) into a scar, how would you go about coding this and which admin code is appropriate, 11900 or 96405?

Wiki Mohs performed by one provider, closure performed by another

dx for both is the C44.222. I'm assuming no modifier would be required since they are technically 2 different specialties

If one dr performs the mohs surgery and then plastics does the closure on the same date of service and are in the same office. Does this require a modifier? The dr would bill the mohs on one claim with him listed as the primary biller and provider. Then plastics would have his own claim with the...
