    Wiki 96127 with 90833

They can be billed together if documented correctly. Time can't be used for determining the level of E/M service, it has to be based on MDM. The time spent on the E/M service can't be included in the time for they psychotherapy service. Refer to CPT book it explains everything.

When a provider completes the PHQ-9, GAD-7. and WHODAS, do you give credit under 'review of the result for each unique test'? The provider is billing with 90833 so the e/m code has to be based on MDM. The provider bills for the forms, they don't always get paid. It seems like if credit is given...

    Wiki Depression Screening G0444

Are they billing and E/M visit with it? If they are, the time for the e/m can't include the time spent going over the depression screening since it is a separately billable service. I would be careful of templated time statements. If the time is the exact same for every patient that can send red...

    Wiki Infusion Coding Question: non-facility

Thank you Thomas. It is through one IV line. I am not able to talk to the nursing staff. I am coding from the insurers prospective on whether the documentation supports the billing. We do not a separate policy in this case, we go off CPT.

    Wiki Infusion Coding Question: non-facility

I have a question about infusion coding. The MAR has the following (I made the chart to make it easy to read): I am confused on how to code it. The initial would be the Ketamine 96365 x 1, 96366 x 3 There is not enough information in the chart note to indicate if the lido and mag were in a...

Normally PTA's services are billed under the physical therapist. Are you talking about the time they spent performing one-on-one services with patient? Feel free to list an example and I can help you or message me directly. I would suggest checking the APTA for supporting documentation.
