I anyone having a problem getting CPT code 99417 to UHC? Per their policy it is a valid code however their claims scrubber kicks the claim out before it makes it to adjudication when the CPT code is on the claim saying it is not an appropriate code when billed with office visit E &M code.
Established pediatric patient, f/u hematuria, proteinuria, mild, duration 1 month, urinalysis done in office, suggested f/u labs, no rx, no other tests. This appears very straight forward. Would this support a 99212 or 99213?
I have been told there is a way to bill for Cerumen Impaction removal using an instrument. Does anyone know of a special code to use for this? How do you bill for this service with an office visit?
