Wiki Diagnosis for billing G2211 (Longitudinal care)

Can G2211 be billed with any diagnosis (for example, a wellness code)? Or does it have to be billed with a problem diagnosis or specifically a chronic condition diagnosis code? If anyone has had denials, I would appreciate hearing of this. Thanks.

Wiki Can G2211 be billed by providers other than the patient's Primary Care Provider?

I am in a Primary Care Practice with many providers. Full rules for using G2211 have not been published, so far as I can find. I can bill G2211 if I see the patient for an acute issue rather than a chronic issue. I would like to hear that Medicare allows an APP or another physician to bill...

Wiki Billing for B Complex injections

I would like to offer B Complex injections in the office. I have not found the right J code for this. Is it appropriate to bill J3420 (B12 injection) for this injection. B Complex injections contain B12 as well as other B vitamins. The description of J3420 is "B12 injection, up to 1000 mcg."...
