Wiki Coding cpt 45390 and 45385

Thank you for your response afalcon :). Could you please shed a little light on why you think 45390 should be coded over 45385? Is it because of the RVU values. If so, should we consider RVU values over column 1/column 2 concepts? Or am I missing something here?

Wiki Coding cpt 45390 and 45385

Thank you for the response MarylouT :). Are the reasons stated in my original question, especially the column 1 and 2 concepts, sufficient to support the selection and sequencing of the codes or Am I missing something here?

Wiki Coding cpt 45390 and 45385

Hi, I've two questions regarding these codes: 1. If both CPT 45390 and 45385 have been performed on the same lesion, should we code only CPT 45385 since CPT Code 45385 is column 1 and CPT Code 45390 is column 2 code. Also, CPT Manual instruction states, "Do not report 45390 in conjunction with...

Wiki Subluxation of tendon - icd-10-cm code?

Hi, How would the subluxation of tendon of right hand, be coded. I'm thinking of going with other specified disorders of tendon right hand (M87.643) as there is no documentation of trauma. Any inputs anybody? Thanks! Amber

Wiki Stereotactic Localiztion followed by Partial Mastectomy

Hi, All the codes seem fine except the stereotactic needle localization code 19283 which shows edit with 19301. I also get these kind of scenarios all the time but in my case the practitioner performing the localization procedure is different from the practitioner performing the lumpectomy and...

Wiki ICD-10-CM Code for malunion of deltoid ligament right ankle

Hi I think I've brainstorming stuff this time. The patient was in an accident (traumatic). This is the first time that this malunion of deltoid ligament of the right ankle has been observed. There was no documentation of earlier repair that could've resulted in such malunion. Were it a bone...

Wiki op note dx vs path report dx

Hi, I've a case where the encounter is for screening colonoscopy (ICD-10-CM: Z12.11). The surgeon found a polyp (ICD-10-CM: K63.5) in the transverse colon and excised it using snare (CPT 45385). Pathology report comes a few days later and states the excised tissue as "normal colonic mucosa"...

Wiki Op report vs path report - dx & cpt?

Thank you for sharing Carol :). So, the "purpose" is key here. If the purpose is to remove sample just for pathological analysis, then, biopsy code would apply otherwise not.

Wiki Op report vs path report - dx & cpt?

Thank you for the response Thomas :) Your response is logical. Actually, I'm confused because if the surgeon excises a tumor and pathology report confirms it as benign or malignant, it's then coded (both ICD-10-CM and CPT Codes) depending on the final diagnosis from the pathology report...

Wiki Surgeon states polyp pathologist states normal

Thank you for the response Lady T :) Actually, the pathology reports that I've received for such cases often mention the terms such as "Hyperplastic polyp" or "Adenoma". If the sugeon states polyp and pathologist states adenoma, I code adenoma (It's just like if surgeon excises a tumor and...

Wiki Op report vs path report - dx & cpt?

Hi, I've a case where: Operative report mentions Wrist volar/antebrachial fascia cyst and H&P report mentions "It was a source of local discomfort left wrist". The patient also has Carpal tunnel syndrome. The surgeon made an incision for CTR procedure and then extended it proximally to gain...

    Wiki Coding denervation

Need help with CPT and modifiers for the following scenario: Denervation of branches using transection from: 1. Superficial radial nerve 2. Lateral antebrachial cutaneous nerve 3. Palmar cutaneous branch of median nerve As far as I understand these should be coded using CPT Code 64772. Now...
