No it is not a verruca, just a callus. For some reason, lately the providers are applying the acid to the calluses after paring. The auditing company is telling us to use 17110 for this but then it gets denied since we are using the diagnosis L84.

Wiki I have an issue trying to get Humana to pay multiple fat grafting units

I have only billed the 20926 code once for each breasts, so if both breasts are done at the same time, then I submit code 20926 with qty 2. It is the understanding from my plastic surgeon and I that no matter how many times the fat is injected into the breast, it can only billed once.

Rearrangement What about using codes like 14000, 14001 or 14301,14302 depending on the size of the tissue rearrangement? I would recommend keeping an eye on that claim, if it gets denied then maybe try of these codes.
