    Yes, thank you, I know this, but does that mean he can use another physicians review of systems? per CMS, it states, ancillary staff or the benficiary. It does not clarify. I have been told in the past that a physician can not refer to another physicians note.

    I have an H&P where the patient was admitted through the ED. The H&P physician has referenced the ED report for the review of systems. Can this be counted if I pull the ED report to verify, although it was done by an ED physician ?? Please help.

    history DG: If the physician is unable to obtain a history from the patient or other source, the record should describe the patient's condition or other circumstance which precludes obtaining a history. this is found in the CMS E&M Evaluation Guide. on page 29.

    auditing prescription drug management Hi, could someone tell me when E/M code auditing under risk and prescription drug management....does this mean it can be counted IF the physician states, continue current meds, OR can it only be counted if a new prescription is given..??

    Hi, I have a scenario in question--pt came in-dx pneumonia--e/m code 99213 ---j0696 inj. given with 96372 --use 25 mod on e/m. next day pt returned. charges were the same--the ins. denied the 99213 but pd j0696 and 96372 on second day. plz advise !!

    Had a pt who came in for e/m and had a removal same day. This pt came back the next day, within the 10 day global, and had an antibiotic inj and xray with this same dx. Can i bill this next day inj and xray? Thanks for your help. !!
