Wiki PA Documentation Do's and Don'ts

I apologize if this is not the correct location for this post. We have an Ortho PA (PA 1) that wants to dictate a discharge summery for a patient that was seen by a different PA (PA 2), the patient was never seen by PA 1, only the Dr. and PA 2 saw the patient. PA 2 discharged the patient, not...

    Wiki Infant death after birth

Not sure how to code this one; Infant had HR of 120 during onset of labor, dropped to 70 and then 0 during birth. Infant recieved CPR, intubation etc for 24 minutes before being pronounced dead. I'm leaning toward 669.93 however my coworker feels that there is a better code.

    Wiki Blunt Trauma - Abd

Running into an issue with coding this one. Patient fell from a ladder, landing directly on his back and side. He complains of left sided lower back pain as well as left sided abdominal and flank pain. No N/V hematuria etc. Provider gives a diagnosis of blunt abdominal trauma, I wasn't...

    Wiki Mystery Lab

I have two labs with a diagnosis of Possible MRSA carrier. (cultures are negative.) Obvisouly I can not code this due to "possible" being in the mix. The problem that I am running into is that none of the providers involved, or that may have been involved, with these patients are claiming...

    Wiki PA documentation trumps MD?

There is no need to apologize for your 2 cents, I agree completely. ^^ If there are any suggestions as to websites that I can look things like that up it would be very helpful, I don't have a lot of luck asking my coworkers about things like this since I seem to be the only coder here.

    Wiki PA documentation trumps MD?

When my supervisor makes herself available for such a discussion I will happily do such, however most of my contact with her is through post it notes stating that I did it wrong and to do it her way. Thank you for your time in replying.

    Wiki PA documentation trumps MD?

The MD was the one that did the final report and documented the absence of the kidney. I'm wondering if V45.73 should be reported in place or in addition to 753.0

    Wiki PA documentation trumps MD?

A patient came into the ER, evaluated and treated by the PA. Patient had a CT scan, read by a MD. MD documented an absence of the left kidney, PA documented congenital atrophied left kidney. Per my supervisor I should code the atrophied kidney and NOT the absence of the same kidney, because...

Wiki Does any one else think it is wrong?

This is true. However it would more of an ethical issue then a matter of fairness. If they're not above cheating on a employment test, what else would they see as harmless to do?

Wiki Does any one else think it is wrong?

I agree as well. The only exception I can come up with is if they were changing specialties and were wanting pointers on what to brush up on.

Wiki Acute to Swing discharge/transfer diagnosis

Patient was admited to acute care with a diagnosis of uncontrolled diabetes, then transfered after three days to swing bed. The Provider dictated a combination transfer and discharge note for both stays. The diagnosis used was 250.00, Type II or unspecified type, not stated as uncontrolled...

Patient came in and recieved Norflex, Toradol and Dilaudid. The provider gave a second Dilaudid IM (96372) 1.5 hours after the first, can this be counted seperately?
