Wiki Vent Dependence

My organization considers Z99.11 a status code. If a patient is discharged on a vent and has not been weaned + there is documentation in the note that vent is still being utilized we can abstract the code. Keywords would be continues on ventilator, unplanned or extended mech vent, currently...

Wiki Atrial fibrillation with pacemaker

Hi all, My office is having an ongoing debate as to whether it is appropriate to code Atrial fibrillation (427.31) when a patient has a pacemaker (V45.01). The Afib is not a problem at this time. Any thoughts? thanks in advance...and if you could direct me to any resources that would be great!

I would use 250.8x and 454.0. Code 250.8x is for "other" specified manifestations so I would feel comfortable coding it this way as long as the physician is linking the two.

Wiki Complete Heart Block with Pacer

Hi - I'm working on a HCC project. I am wondering if it is appropriate to code a complete heart block when a patient has a pacer (placed because of blockage). The block has an HCC but the pacer does not. What do you think? Thanks in advance!
