On a practicode case, I coded LT-22, and got dinged for stating them in the wrong order. The rationale says the modifiers are 22-LT. Would my code get rejected like that in real life? Are we required to state the level I modifier first and level II second?

Per the guidelines: If more than one modifier is needed, list the payment modifiers - those that affect reimbursement directly - first: Payment Modifiers included: 22,26,50,51,52,53,54,55,58,78,79,AA,AD, TC, QK, QW and QY
In the above Modifier 22 should be 1st and LT should be 2nd Modifier.

Modifiers that change payment are listed first. Descriptive modifiers follow. An insurance would not process this line with -LT -22.
